What is WIC?

The Special Supplemental Nutrition Program that provides nutrition counseling, breastfeeding support, and an eWIC debit card to purchase nutritious foods for eligible Women, Infants, and Children (WIC).

Am I eligible?

If you meet these categorical, residential, and financial requirements:
Categorical
If you or someone in your household is:
  • A pregnant woman
  • An infant or child up to 5 years old
  • Mother of an infant up to 6 months old or breastfeeding mother with an infant up to 12 months old
  • A father or caretaker applying for their children
Residential
Applicants must be a resident of New York State, but you do not need to be a U.S. citizen.

Financial
  • Applicants who receive Medicaid, SNAP (Food Stamps), or TANF (Temporary Assistance for Needy Families) qualify for WIC
  • If none of the above, applicant must meet income eligibility guidelines (see table below)

What Do I Need to Apply?

Medical Referral Form
This form will need to be completed by your doctor.
WIC Medical Referral Form
One of These Proofs of Address
  • NYS Medicaid card
  • Current rent or mortgage document
  • Current pay stub with address
  • Utility Bill (electric, gas, etc.)
  • Photo ID with current address
  • Letter from shelter/house of worship/social services on their letterhead
One of These Proofs of Income
  • NYS Medicaid card
  • Food Stamps letter
  • TANF – public assistance letter
  • Most recent pay stub
  • W-2 form
  • Employment letter
  • Unemployment book
